If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
We are pleased that many patients have dental insurance plans. Policies covering dental treatment vary widely in the types of procedures covered, and the amounts they cover. No dental insurance plan covers all dental costs. Our office will be happy to help you in obtaining a pre-estimate of proposed treatment, but sometimes they do not guarantee coverage. Any portion of the fees not covered by insurance is the responsibility of the patient at the time of service
Most dental insurance plans have fee allowances and deductibles. Payment for the differences in fee allowances and deductibles is expected at the time of treatment. If there is any balance due after the insurance is filed, the patient will be billed for the difference. If any overpayment is made to our practice, the credit will be mailed to the patient.
